Board's Online Registry

1  All final disciplinary actions and only non-disciplinary orders within the last five years are shown in the search results. The Board office should be contacted to determine if the licensee has a history of dismissed complaints, advisory letters letters of concern or non-disciplinary orders.

2  The Board assumes no responsibility for errors or omissions contained in this directory. Therefore, a licensee's status, history, and address should be verified with the Board office. Furthermore, the presence of an individual listed in this directory does not in any way constitute an endorsement by the Board of Chiropractic Examiners.

All Chiropractic licenses, unless renewed, expire on December 31st every year.  An ACTIVE status means a license is not expired.  An Administrative Suspension (ADM SUSPENSION) status is imposed when a licensee fails to timely and accurately renew their license. In accordance with ARS § 32-923(H) "administratively suspend" means a non-disciplinary action that is imposed for failure to renew a license and that requires the licensee to suspend practice until renewal requirements are met.
