Continuing Education-Providers

The Arizona State Board of Chiropractic Examiners welcomes your interest in providing continuing education for Arizona licensees. Please read the following forms, laws, and rules carefully before submitting your application, as the Board cannot refund application fees, even if you do not qualify for approval.

Applications are processed in the order they are received. Please allow staff at least 60 days to review and process your application(s) and issue your AZCE Number(s). The Board has the authority to approve continuing education courses if needed retroactively. 

The Board no longer accepts handwritten applications; you must submit your application using the online applications on this page. 

When submitting payment by check or money order, please use a single check and the Board's CE Application Check Log Form. The Board will need to receive your payment within ten days of your CE Application, or your application will be closed, and you will need to reapply. You may pay for your Continuing Education Applications by credit card through the Board's Online Payment Portal

Board Approved Trainer

If you wish to be able to provide Board Ordered Training to licensees subject to non-disciplinary or disciplinary action, please use the Approved Trainer for Board Ordered Education listed below.  The Board is seeking training in the following areas:  ethics, billing, x-ray imaging, AZ Jurisprudence, record keeping, history, diagnostic procedures, and evaluation/management. Please review the necessary instructor and training criteria included in the application. Chiropractic Assistant Training Courses

If you wish to provide training courses for Chiropractic Assistants as staff members in your office or provide a Chiropractic Training Course as a course offering, please use the Chiropractic Assistant Training Course Application listed below. Providers must provide a minimum of 24 hours of education in the following topics: Chiropractic Principles, Management of Common Diseases, History Taking, Record Keeping, Professional Standards of Conduct, and CPR. Chiropractic Assistants supervised by a chiropractic physician with specialty certifications must complete additional education in the specialties (PMMTP and Acupuncture). The Arizona State Board of Chiropractic Examiners welcomes your interest in providing continuing education for Arizona licensees. Please read the following forms, laws, and rules carefully before submitting your application, as the Board cannot refund application fees, even if you do not qualify for approval. The Board will NOT approve courses in which the primary focus is the promotion of a product or service or courses that require participants to purchase a product or service. Each course shall be on subjects of CLINICAL BENEFIT TO THE CONSUMER OF CHIROPRACTIC SERVICE. The course must have a significant relationship to the assessment, diagnosis, or treatment of patients within the chiropractic practice scope, as defined in A.R.S. § 32-925 and 32-922.02.

Each application must include the following: 

  • A completed Continuing Education Course Application. (You must complete one application for EACH course, whether individual or included in a program of multiple courses, i.e., conferences, seminars, online course packages, etc.)
  • A $ 50.00-course application fee for each course, whether an individual course or included in a program of multiple courses. The Board must receive a credit card, check, or money order within ten days of submission. Staff will discard your application should you fail to submit your payment. If you apply for in-person and online instruction, you must pay a $50.00 fee per method (In-Person $50.00 + Online $50.00=$100.00). You will be issued a different AZCE Number for each method.
  • You must include a resume or curriculum vitae for each course instructor.
  • One letter of reference for each course instructor from someone familiar with the instructor’s qualifications, education, or experience in the course subject.
  • Verification of licensure in good standing from each jurisdiction where each instructor holds or has held a license. VERIFICATIONS CANNOT BE MORE THAN 30 DAYS OLD.

You must submit your complete application 60 days before the course's anticipated initial offering. The Board will only accept applications submitted electronically. Handwritten applications will NOT be accepted. 

Subject Eligibility:
To qualify for Board approval, a continuing education course must meet the subject requirements outlined in A.A.C. R4-7-801 (D), (J), or (K). Course subjects that DO NOT qualify for continuing education credit are listed in A.A.C. R4-7-801 (L). Please read these lists carefully before you submit your application.

Instructor Eligibility:
Instructors must be qualified by education and/or experience to provide instruction in the course subject matter.

Qualifying instructors must:

  • Be faculty of an accredited college or university or demonstrate equivalent qualifications through postgraduate study and experience teaching postgraduate coursework.
  • Hold an applicable license in good standing.

Qualifying instructors must not:

  • Have had a license placed on probation for unprofessional conduct within the past five years.
  • Have had a license suspended, surrendered, or revoked.
  • Have had an initial license or renewal application denied for unprofessional conduct.
  • Have been convicted of a felony.

Online Courses:

  • Pursuant A.A.C. R4-7-801(C)(4)-The Board must preapprove online instruction, and the course must be offered or sponsored through an accredited college or university.
  • If you provide the course in an online or webcast format, you must complete the Online Addendum and provide a notarized copy of the sponsorship agreement.

Not all continuing education courses require Board approval. The following providers and courses are predetermined to meet the continuing education requirements. They do not require, nor should they expect, Board approval. The Board will return all applications submitted for courses that meet the following criteria without review.

  • In-person courses provided by accredited colleges and universities, with qualified instructors, on topics allowed by statute and rule. (This does not include sponsored or co-sponsored courses or those taught by an adjunct faculty member of an accredited college or university.)
  • In-person courses provided by the American Chiropractic Association and the International Chiropractors Association, with qualified instructors, on topics allowed by statute and rule.
  • In-person CPR training provided or sponsored by the American Heart Association (AHA), the American Red Cross, or an entity that meets the equivalent standards of the AHA and the Red Cross as determined by the Board.

Approval is required for providers not listed above and for ALL ONLINE COURSES.

CE PROVIDER APPLICATION

ONLINE COURSE ADDENDUM

 You can submit your payment online using a credit or debit card. You are able to submit payment for eight (8) applications per credit card transaction. You will receive a receipt with your authorization number. Please include the Credit Card Authorization number on each CE Application. 

ONLINE CE APPLICATION CREDIT CARD PAYMENT

The State of Arizona Board of Chiropractic Examiners accepts PACE recognition as satisfying the Board's requirements for the Continuing Education process. However, courses must still be individually approved by the Board. The Board will NOT approve courses in which the primary focus is the promotion of a product or service or courses that require participants to purchase a product or service. To receive full CE credit, attendance data must be uploaded to the PACE database within 30 days of course completion. Additionally, your course will not be approved if it is not listed in the PACE Course Catalog; your application will be closed, and you will have to resubmit your application.

Credit Card Payments

You can submit your payment for your CE Course Applications/PACE Pre-Check Applications using a credit or debit card for up to eight (8) applications per transaction. The State of Arizona charges a $4.50 credit card convenience fee per transaction. Application and Convenience fees are non-refundable. 

You will be emailed a receipt with your credit card authorization number. Please include the Credit Card Authorization number on each Application.

ONLINE CE APPLICATION CREDIT CARD PAYMENT

 

After a licensee has completed your course, you must provide them with a certificate confirming course participation.

  • The certificate must provide the name and license number of the attending chiropractor,
  • The name of the course provider,
  • The course approval code,
  • The subject matter of the course,
  • The course title (if different from the subject matter listed),
  • The date and location of the course and
  • The number of hours of continuing education the chiropractor completed.

Course Approval and Expiration Information:

If the Board approves your course, you will receive a letter providing you with your approval code. To maintain your approval, you must reapply every two years by the first day of the month the course was initially approved. You must also reapply every time the course subject or instructors change. Failure to reapply will result in the disqualification of your course.

Course Certificates:

After a licensee has completed your course, you must provide them with a certificate confirming course participation. The certificate must provide the name and license number of the attending chiropractor, the name of the course provider, the course approval code, the subject matter of the course, the course title (if different from the subject matter listed), the date and location of the course and the number of hours of continuing education the chiropractor completed.

Compliance Requirements:

As the provider of a Board-approved continuing education course, you are under a continuing obligation to report to the Board. You must notify the Board immediately of concerns or problems that arise regarding your course, including any disciplinary action imposed on the license of an instructor or an instructor being convicted of a criminal offense.

Maintain a List of Attendees:

In accordance with A.A.C. R4-7-801 (H)(2) and (3), you must maintain a list of all course attendees for a minimum of five years after each date the course is held and provide a copy of the list to the Board within ten days of a written request to do so.

Maintain Course Information:

You must also maintain a copy of the course syllabus with stated learning objects, a list of instructors, and documentation of the name, location, and date of each course held for at least five years. You shall provide a copy of these materials to the Board within ten days of a written request.

Board Audits:

A Board representative may attend your continuing education course to verify its content and ensure its compliance with statutes and rules at no charge. The Board may withdraw its approval if they find that the course provider is not compliant with statute or rule, has misrepresented the course content, or has failed to obtain consent after changing the course content or its instructors. By applying to the Board, you agree to comply with these provisions.

When submitting your electronic applications with a check or money order for payment, the Board requests you submit a single check with this form to track your application(s) and payment. Please ensure your CE applications include the check or money order number in the payment section of the application. 

Please note that the Board needs to receive your check or money order within ten days of receiving your electronic CE Applications. If payment is not received within ten days, your application will be canceled, and you will need to resubmit the application packet.

STATUTE AND RULES FOR CONTINUING EDUCATION

 

The State of Arizona Board of Chiropractic Examiners welcomes your interest in becoming a Board-approved trainer to provide training for licensees that are subject to either non-disciplinary or disciplinary action.

You must meet the following criteria:

  1. Must hold a degree from an accredited college in the applicable profession.
  2. Must hold an active and pertinent professional license in the applicable jurisdiction.
  3. Must have obtained the appropriate specialty certification/additional professional credentials required for a particular specialty.
  4. Must use a set of generally recognized evaluation criteria.
  5. Demonstrated experience/understanding of administrative law, evaluation of fitness for duty of the health care professional, and safe-to-practice assessments for health care professionals.

You will receive notification of the Board’s decision in writing. You may not commence any Board ordered training before receiving approval from the Board.

Board-Approved Trainer for Board-Ordered Education Application

The State of Arizona Board of Chiropractic Examiners welcomes your interest in providing training for Chiropractic Assistants. 

Application Information and Requirements:

  • Applicants must not currently be the subject of disciplinary action.
  • Licensees subject to disciplinary sanction do not qualify for approval as an instructor of a chiropractic assistant training course for three (3) years from the date their order was terminated.
  •  Attach the course syllabus and printed instructional material: Chiropractic Principles, Management of Common Diseases, History Taking, Record Keeping, Professional Standards of Conduct, CPR.
  • Chiropractic assistants supervised by chiropractic physicians with specialty certifications must complete additional education in those specialties (PMMTP or Acupuncture).
  • A Resume or Curriculum Vita for each instructor.
  • A Sample Attendance Sheet.

The Board must approve your training course before you can provide it to chiropractic assistants. Once approved, the Board will issue a course identification number to you. Chiropractic assistants will be required to provide this number to the Board, on the Chiropractic Assistant Registration form, upon completion of their training.
