Inactive Retired Status Change Request Form

To change your license status to Inactive or Retired, your license must be in good standing as per A.R.S. §32-933(A), and you must no longer be actively practicing chiropractic in the State of Arizona. To request this status change, complete the status request in your Thentia Portal and submit this required form and affidavit as supporting documentation.
